How to Get Started

Here's how to get Increlex treatment under way (if you haven't taken these steps already).

  • After you are diagnosed by your doctor and Increlex is determined to be an appropriate therapy, you and your doctor must fill out a Statement of Medical Necessity (SMN), and a patient consent form in order to qualify for insurance and take advantage of all the support our unique TerciCareSM program offers. Read the SMN carefully, make some choices, and sign it. Also read your TerciCare brochure to learn more about the help that is available.
  • After the SMN is faxed in by your doctor, you may, if appropriate, qualify to receive a FREE Increlex Travel Kit with a FREE 30-day supply of Increlex. See the TerciCare brochure for details.
